    The forms listed below are for the convenience of church pastors. If you are a pastor or church employee needing forms related to the annulment process, you may download them here. NOTE: These forms are for downloading by pastors, their representatives and church staff. Individuals interested in church annulments should meet with their pastors for forms. The Tribunal will not accept a Petition for Church Annulment from an individual. The pastor or his designee must be a part of the process. These require Adobe Reader as they are .pdf files.
